A service for my child due to serious behavioral and/or emotional distress (i.e substance misuse, severe acting out, serious emotional distress/dysregulation)

Crisis Assessment - I feel my child is in IMMEDIATE danger; talking about hurting themselves or someone else.
Substance misuse - My child is using drugs and/or alcohol
Behavioral/emotional Distress -My child has serious acting out behaviors (not following safety rules, destroying things, hitting or threatening) or signs of severe emotional distress (self harm, severe anxiety or depression, angry outbursts).
Care Coordination to help access/match/connect to services for my child\\\\\\\'s complex behavioral health need

MILD - I am concerned about my child using drugs and/or alcohol, but the use is not frequent and/or there have not yet been any series consequences.
MODERATE My child\'s use of drugs or alcohol is causing problems in at least one area (at school, with peers, at home).
SEVERE - My child is using drugs and/or alcohol frequently and this use is creating problems in multiple areas, such as school attendance or performance, relationships with peers or family, engaging in illegal activities, engaging in very risky behaviors. I am afraid my child may not be able to stop using if they wanted to.
MILD Child does not have prior treatment history and/or symptoms have limited impact on daily life
MODERATE Child has symptoms that negatively impact daily functioning
SEVERE Significant psychiatric diagnosis and/or youth at risk of out-of-home placement/hospitalization or is being referred from hospital/out-of-home placement
